Presurgical Requirements

Once your Initial Consultation has been scheduled, you will be required to complete a series of Medical Clearances.

These clearances may include:

    Primary Care Physician Clearance

    EKG

    Chest X-Ray

    Psychological Evaluation

    Upper GI exam

    Gallbladder Ultrasound Exam

Also, depending on a patient’s medical condition or insurance requirement, additional Medical Clearances may include:

    Cardiac Clearance

    Pulmonary Clearance

    GI Clearance

    Medical Weight Loss Management

    Nutritional Evaluation

All requirements will vary depending on each patient’s individual medical condition and insurance plan. After your consultation with Dr. Douglas Krahn, you will sit down with a qualified professional who will explain your individual needs.

Self Pay Options

We offer competitive cash rates to those patients who do not have insurance or those who do not have bariatric benefits with their health plan. Financing is available for the patients who are self-pay and for PPO patients who have out of pocket expenses associated with their policy.
Please consider Care Credit to assist you in your financing needs. An online application can be completed at www.carecredit.com.
Our finance department can help you with all the questions concerning cash pricing, insurance, out of pocket expenses, and financing.

Billing

At Western Bariatric, AMC all billing is done In-House, any questions our patients might have regarding claim status, deductibles, patient responsibility, out of pocket cost, co-pays, or any other miscellaneous fees can be answered either by phone or in person while visiting our office. A variety of different Health Plans are accepted including HMO’s and PPO’s. Please contact our office today with questions or inquiries regarding your insurance.

Insurance Options

There are a number of different plans available to a patient, and at times, it becomes very confusing and difficult to choose which one fits your needs. Bariatric patients are required by their Insurance Company to undergo certain requirements prior to being granted surgery authorization. Each health plan has different criteria, but most are similar to those listed below:

    A BMI of >35 with two co-morbidities (listed below) or a BMI of >40

    Medical History of Co-Morbidities associated with obesity (Diabetes Mellitus, Obstructive Sleep Apnea, Hypertension, Gastric Reflux Disease, Degenerative Joint Disease/Arthritis, Hypercholesterolemia, etc.)

    Medical history of Obesity for up to 5 years

    3-6 months weight loss management (either by PCP/ Dr. Krahn/ Weight Loss Clinic) participated in within the last 2 years

    Nutritional consultation and assessment

    Psychological evaluation showing the patient is willing to comply with a strict post-operative regimen

    Age ranging from 18-65

Not all insurance companies require the above criteria, although they are commonly seen in most. Our office staff is very knowledgeable and willing to assist you with understanding the insurance criteria of your health plan.

Support groups are an excellent resource. You’ll find people who share your goals of health and wellness. Our support group is a forum for celebrating successes, such as the improvement or resolution of co-morbidities. Our support group is devoted to people who have common experiences, who can share their feelings in a safe environment, and develop relationships that can contribute to improved physical and emotional health.
